Release
By signing below I am voluntarily participating or voluntarily allowing my child(ren) to participate in sports activities with Basketball Tao Dojo, Basketball & Beyond, Brian Howard, and all staff collectively (“Basketball & Beyond”), including but not limited to basketball training, conditioning, and other sports at 1980 S. Quebec, and any other locations for training, clinics, games, and any other basketball, related activities including strength and conditioning (the “facilities”). For in and consideration of access to and use of the equipment, events or activities of Basketball and Beyond, I hereby relinquish, discharge, and release Basketball and Beyond, it’s owners, employees, agents, independent contractors, consultants, now and forever, from any and all claims, demands, liability or causes of actions for personal injury, property damage, or death occurring to me or my children(ren) or to anyone else caused by or arising from my presence at or use of Basketball and Beyond facilities, equipment, events or activities, including but not limited to injury or damage caused by negligence of Basketball & Beyond or the owners, operators of the facilities, now and forever, from any and all claims or causes of actions by whomever or wherever made or presented caused by or arising from my presence at or use of the facilities, equipment, events, programs or activities of Basketball & Beyond. It is my understanding that there are risks of minor, serious or permanent injury or death in any recreational or sport activity and we voluntarily assume such risks. I hereby
acknowledge, understand and assume the risks and potential dangers arising from the conditions and use of the facilities, equipment and premises, and participation in Basketball & Beyond events or activities. I acknowledge and understand that included in the scope of this waiver and release is any cause of action arising from the performance or failure to perform construction,
maintenance, inspection, supervision, or control of the facilities, equipment or premises. In the event there is an emergency and I cannot be reached, I hereby consent to whatever medical treatment may be considered necessary by the attending physician for me or my child(ren). In addition, I agree that in the event that my child(ren) and I are inured as a result of participation in Basketball &
Beyond events or activities, whether or not caused by the negligence of Basketball & Beyond and any of its owners, employees, agents, independent contractors, and consultants, recourse for the payment of any resulting hospital, medical, or related costs and expenses will first be had against any hospital or medical insurance or any available benefit of mine and my family.
